I'm almost certain crazy eyes is a morph of the band members' faces. The left eye looks like Mike's, the right being Axe's. The nose is Mendez's and the mouth is Fredrik's. I can't seem to figure out where Per fits in, though. ... or not.

The DVD is awesome. I've only ever bought two concert DVDs - Lamentations and Wrath of the Norsemen. Both were well worth the money.
