Company in the US that prints CD covers?

I actually want the cardboard type cd covers that open up. Where is a good place in the united states to get this type of service done for say...200 of them? Also what about actual CDs. I want the CDs to be printed on as well.

I've worked with Disc Makers twice, recently with full on jewel cases for my band's EP, and I did those cardboard sleeves a few years ago with a previous band, it was like 1,000 of them for $990. When you buy less, say 200, it will still probably cost you like $700 or so (just my estimate) so that's how they rope you into buying the 1,000. Anyway, they're really good quality...glossed if you want for no charge, great printing, shrink wrapped, real CDs (not CD-Rs) and everything. Also, great customer service...they assign you a specific project manager who is very knowledgeable and easy to contact directly. I honestly have nothing bad to say at all about my Disc Makers experiences!
