Holy shit. That's a killer lineup.
The core of Adam's sound has always been an old Marshall Superbass. It's a four input, non-master volume Marshall head from the early '70s; I think it's a '71, if I recall correctly. It's somewhat modded, but as far as I know, the channels are just jumpered internally rather than running a patch cord from the second input of channel I to the first input of channel II. It's run through a Marshall cab with Celestion G12H30s. And turned up. All the way.

I've heard clips of a cranked Superbass before, and they're most of the way there as far as Tool's tone is concerned. It's just an old Marshall, very, very, very loud.
IIRC, Opiate's just the Superbass. I don't recall if he added the Dual Recto on Undertow or if he didn't have it until Aenima. Anyway, it's just a two-channel Dual through a Mesa traditional 4x12 with V30s. It's turned way up and blended with the Marshall, and that's about all I know.
Same deal with the Diezel VH4s. He has two. He was Diezel's first US customer; his blueface VH4 is the first one ever imported into the US. He's got a more recent silverface VH4 as well. The Superbass/Recto/VH4 was the core of his live rig for a long time. Somewhere along the line, the Lateralus tour, IIRC, he dropped the Dual Rec for an Uberschall for a bit, then settled on a second VH4.
Oh, and he used a Sunn Beta Lead on Lateralus as well.
DSS3 is right about the 10,000 Days setup. That's what I've heard as well. 10,000 Days is a bit different, tone-wise, 'cause you can pick out different tones for different songs, and even different sections of songs. Usually all his stuff is just going at once and blended together. They mixed and matched amps on 10,000 Days. You can hear the change from Vicarious to Jambi, for example.
