Theres a pic of him with his bass in the Prelude to the Millennium booklet, although I can't see the headstock or anything.

And yes its a 5 string.

In the first pic it looks like warwick thumb, but in the second the headstock looks kinda like carvin.

I'm pretty sure he did some shows in Japan with them. A long time ago I remember hearing a bootleg from Osaka and Russ introduced him with the rest of the band.
