Narcotic can refer to two things:
Narcotic = A drug (ei: narcotics...)
Narcotic = Someone who is on drugs...

Psychotic intellectual narcotic (or even narcosis, as it's the same derivative)-
Psychotic refers to the result of a psychosis, which is pretty generic in modern psychology terms, but it basically breaks down to the effect of insanity. Usually in a destructive and violent manner.
Intellectual means dealing with the intellect, intelligence and reasoning.
Narcotic is a drug a drug user, as said before.
So, a psychotic intellectual narcotic would be a drug that induces violent and even pathological intelligence. Perhaps it implies the result of such high intelligence/awareness naturally induces a violent response to reality, since reality sucks. Or perhaps that, a person who is so intelligent, would be seen as psychotic by others because his/her line of thinking would so vastly supercede everyone else's. Personally, I think it's a reference to good fuckin' metal. Dark, more intelligent than pop shit, and often with tinges of aggression and violence, or psychosis. Metal is a psychotic intellectual narcotic.
