anyone know where to get EoR remix in slipcase?

I remember century media saying the EoR Sneap remix could be purchased in a slipcase of $5. I can't find it. I found this: http://www.centurymedia.com/temp/ProdDetailfr.cfm?SID=-657132892&ProdID=8110-2 which is really confusing because it says Kelly Gray did the production (unless that's sarcastic).

Anyway, is the remix available for $5? Thanks.
