Contract Template

Until now I've been doing everything by word, I mean never had a contract for the promotor to sign in live concerts. Due to some succesful project I'm having offers to play in many places and people ask me for the contract/conditions.

Could any of you send me a template so I can create my own band's contract based on yours?
