Monitoring setup while tracking?

What's your monitoring setup when you're tracking a band in your studio?

I meant what's the chain, from cables to models of equipment.

for the drummers monitor-mix:
m-audio profire 2626 HPA-out => stereo 6,3mm connector cable to XLR 2m => 30m stagebox => XLR to 6,3mm jack cable 3m=> AudioTechnica ATH M-50 headphones => drummer

there is a little loss of signal due the long calbe from the headphone amp to the drummer, but the amps in the 2626 are powerfull enough so i dont really care.

i create the monitor-mix in reaper (sends from the tracks to an output).

99% of the people here are probably running a stereo out, mixed in DAW, to a headphone amp, then to headphones. There are a few different multi-output headphone amps. Behringer has a real slick personal monitoring system. Which I'm sure they stole from someone else. There's also the Hearback system, one studio I've worked in has that. It's pretty rad, but expensive.
