If you look very closely you can see the Reaper on the left side. This picture is very small. I'm waiting to see it in a larger size to see more details.

Well I guess there's not much to understand, the artist is using different symbols to create a certain impression, it's for everyone to interprete the image in his own way.
That's what I like about it, and besides, it's looking cool

Well, I guess he is "a little bit" broken, mentally broken. And what I mean by this, he has thought about (maybe) suicide and the voice in his head says "You're better off dead." And what comes to Mr. Reaper, he is just waiting for his new victim.

That's all what came to my head, better ideas?

Dethroned: Yeah, this sounds really good. Maybe we will understand it when we know the lyrics to it. It could be that it is a concept between lyrics and the artwork. This would be something new in the COB history.
