if a person sees that you are physically hideous, he will call you ugly because this is his true opinion. This is how he percieves you. Of course it may not be terribly polite, but that is another matter. He isn't just saying that in the context of name-calling, or just to get back at you, or to insult you for no specific reason.

No, if someone calls you ugly, they are trying to insult you, unless they are mentally retarded. Anyone with a little common sense knows that you don't go calling people ugly just because that is your "true opinion." Saying "no offense" doesn't change that.

No offense only works when you're giving your opinion on something that the person can change and even then, you should probably tell them what you don't like and how they could change it instead of just saying "you suck, no offense."

But going back to being ugly, I guess technically you could tell them they are ugly and they should get a different haircut or something, but that's still a mean thing to say unless maybe the person asks you first.
