I created the logos for Thales and Soul Forlorn, yes, although Thales have moved on to a new one (check out
http://thalesmusic.cjb.net), designed by a friend of Hearse's, so the website's layout has been updated accordingly.
It's probably not good business or whatever to discuss publicly too much about the financial stuff, but basically, some of it I've been paid for and some of it I haven't - usually due to the promise of future work for which I will be paid for, an example being that I haven't charged Thales for anything yet, but would like to do their first studio album and be paid for that.
I am trying to make it a policy to charge for client work, at least when it comes to covers for albums that will be sold. It's kind of a dream of mine to be able to make money doing this kind of work (to be the "next Travis Smith", hehe) but at the same time, I am realistic about needing to get my name out there, and the financial situations bands and their members are in. (Of course, this only pertains to the client work as most of the stuff posted here are personal things I really did just for fun)

Some of them I can knock off in an hour or two, others take weeks. It depends on a few variables: if I have the image data, if I have what is to be the "final" concept, what the complexity of that concept is, etc. Taking the necessary photos takes time, as does discussing what the image should be and figuring out how to go about it. If I'm just taking one of my pre-existing photos, tinkering with it a little, and adding a logo and/or some text, it doesn't take all that long at all. For Hammering Moss, I just took some random stuff I was working on for fun and put "Hammering Moss" on it. Five minutes - done. The UM splash page I had to confirm a concept with Mark, take custom photos for, get them developed and scanned, then put the whole thing together - it took a couple of weeks.

Thanks for the praise, Lina, but that's actually exactly wrong. For most of the stuff I do, I look into my archive of photos that I've taken, get them into photoshop, and start experimenting and see what I come up with. There are only 2 images I have ever completely visualized first and then sought out to create: The UM splash page, and the early-completed cover for Soul Forlorn's (yet uncomplete) second album, neither of which are here.
The two best ones I've posted here (IMO) are the Soul Forlorn "A Love So Dark" cover and the "Ambient Domains" disc 1. For both of those, I just started with a picture, and saw where I could go with it.
Here's another example of pure experimentation (although not an album cover). I just ran what ended up being the squarish background part through some filters (to get it that way), then tried adding some photos I hadn't really found a use for yet. When I found a way to put those pieces together... I added some more stuff, got some ideas along the way, then had this.
