Does anyone know what sort of permissions need to be sought for use of OLD (like Renaissance-era) artwork for stuff like album covers?

Furthermore, anyone know what permissions need to be sought for using such a thing, but manipulating it to personal needs on top of it?
 
it should be fine - the original copyright owner isn't able to sue you posthumously (that would be.. weird).

besides, even the notion of copying a painting in this manor didn't exsist in those days

the artist owned the work, and the copyright died with him/her. someone can own the physical painting, of course, but not the copyright.
 
I think it becomes for the domain public( not sure if it's called like that in english) 50 years after the last person with the rights dies.

So you can use it if the author died at least 50 years ago.

At least it's this for music, I would check to be sure but I'm pretty sure it's the same for any kind of art.
