Cubase always "forgets" VST connections...?

Whenever I open Cubase, I always have to re-assign the I/O settings (everything always says "not connected")

Also, anyone else find it annoying that you have to disable an external plugin, as in completely remove it, in order to do an offline render?

I'm still with 5, but you can save I/O settings presets, so in 2 clicks you can restore your full setup :

Yes and yes.

Best practice seems to be to make presets like TRUIE mentioned.

make a blank template with your i/o already assigned in it and use that everytime you start a new project.
