Recommendations for in-ears/earplugs?

I seem to have lost my in-ears, so I need to get some new ones. I was using like a $100 pair from Etymotic. They did a pretty damn good job. Any other recommendations? I'm curious as to how the custom-fitted ones fare, and how much you've paid for them. Bonus if I can get in-ears as well as earplugs (for concerts, etc. I've never found any that fit) fitted from the same place.
 
I have Elacin's/Exinore's. I have compared them with my headphone while listening to music, and they do an incredibly good job.

Whats also great about them is that they are sold with an warranty (for 2 years). If you lose your earplugs, you get new ones.

BTW - the folks at Headfi have excellent information on just about every in-ear & headphone ever made. :)

holy shit, some decent in ears for $20? i'm about to buy some now. i've been looking for some ears to try when doing monitors etc to keep all that stage shit out. work in a really live room.
 
Oh shoot - they dropped, I got em a couple months ago for $26 - now they're $21!
