These may be a little obvious, but they're hard to beat.
I always felt that part of what makes a classic album "classic" is the cover art.
To me it's always a big part of buying new albums.
So eerie even 41 years later. More than just a classic.
Love anything by Repka, but this will always be his greatest work IMO.
And this one just fits the album so darn well, I always imagine it's the thing on the cover doing the vocals.

First time I saw the cover I was like "Please let the music be as good as the cover!" And it was.
Simply amazing. First saw a low quality image of this where you could barely make out the red.
Changed everything when I finally saw it in good quality.
And this is just looks great, I bought the books almost just because of the covers.
And last but not least; Manilla Freaking Road!
Other stuff off the top of my head: Toxik's "Think This", old-school Sepultura, Ozzie's "The Ultimate Sin", Xentrix's "For Whose Advantage" & Savatage's "Hall of the Mountain King".
