Foam after the nut, before the bridge. How large ?

pretty ok, i do it the same way. for the saddle i use this (tesla vibration damper) though...

Move the foam closer to the body string holes (away from the bridge)
I think your bridge is the wrong way around, unless you did that on purpose.
I used to have the bridge on backwards and it would cause the saddles to move when tuning (fucking up intonation)
 
if the screws are facing the neck the bridge is on the right way.

Yea, I think you are partially correct.
However, I have seen new guitars with screws in the opposite position of what you have said.
I don't think it really matters as long as you don't get intonation issues.
