His grandmother died during the production of Deliverance. It's in the session diary for Deliverance/Damnation at the Opeth site. He also dedicates it to her in the back of the booklet for D1.

No, it isn't. Its a place near where I am located called the whaley house. It is a history site actually, very old and supposedly is documented haunted. You can fins stuff about it on the web. It is not much to look at from the outside but inside is kind of creepy. On the street outfront tare grave markers in the pavement because the road was built over a graveyard and the relatives came back later and marked the road after they located the plots on a map. cool shit.
