here's another gaffe i made last month:
i was studying for an exam at a friend's. (you have to know her father died when she was really young). well, we were studying when her nephew arrived; he went into another room and i told my friend "i'm sorry, but in all honesty... i don't really like your nephew's name". she said "it was my father's name"

A few years ago, some friends and I were making prank phone calls to people we knew. My friend dialed a number and handed me the phone and some guy answered. I said something about wanting him to fuck my eye socket. It wasn't til he asked what else I wanted him to do that I realized it was one of my teachers. He knew it was me, I couldn't look at him without feeling stupid for the rest of the school year.
