Where the hell is the Alexi Impersonation "entry form" ??

I've looked everywhere on multiple sites and have done about 40 minutes of Googling.

I asked on the On-Topic forum but the deadline is in hours and I don't have time to fuck around.

Either way they are being misleading either by mentioning some non-existent "entry form" or by not even providing the form. I can't find it anywhere on metalinjection or on the dozens of other pages/forums I've googled.
