Well, in general, its about a guy who dies (Prologue = his funeral)
and then he comes back as a ghost , and watches over his beloved...
The whole thing is about what he feels and thinks when seeing what his girlfriend (or whatever) is doing after he's gone.

It also seems to take place over a year. Starting towards the end of Winter ("Prologue"){"A soft breeze passed me by, somewhat warmer for a second. I knew it was the coming of spring, thus our..."}, and moving into Spring ("April Ethereal") , going on to Summer ("The Amen Corner"){"White summer. So far I have gone to see you again."}, then Autumn/Fall ("Demon of the Fall"){"Silent dance with death. Everything is lost. Gone with the arrival of Autumn."}, then onto Winter ("Karma"){"The cold season drifts over the land...Draped within a fate I could not change, and always welcoming Winter's epilogue} and finally("Epilogue") {"Rising and telling everyone about the beauty of its prologue."}.
