From The Crow soundtrack. I said it before in an old thread like this if you don't shed a tear to this song then you aren't human.
 
Another one for me would probably be Radiohead - How to Disappear Completely. Such a beautifully sad song.

Camel - Rhayader Alone, I'd say this is the saddest of them all
 
Anathema - One Last Goodbye (tearjerking and amazing all at once)
Agalloch - Odal (shows that instrumentals can be sad)
Type O Negative - Death in the Family
 
Johnny Cash - Hurt

There's lots of songs from Katatonia, Anathema and so on that's full of melancholy but Johnny Cash's Hurt is the saddest. There's just something so touching in old man singing such a sad song full of memories close to end of his days.
