What happened to their forehead?

Well that was some kind of weird accident. I was jumping over someone laying on the ground and had no shoes on when that guy suddenly pulled up his head so I hit him really hard. That left a mark on his head for quite a while.

What do they do to completely remove it? Is there something like a "root" of the toenail?

I once hit someone so hard with my foot in the forehead that the nail of my toe
got shoved deep into my nailbed so I had to get a little improvised surgery, too.
