I'm going to fall into the camp of 'If you can't deal with it, go somewhere else.' Some places also offer outside seating, which can be utilized by non-smokers as well.
The problem with that is, everyone needs to breath. No one needs to smoke, and many people choose not to smoke at all. Why should the one making the choice to smoke drive away those who don't wish to smoke?
I can understand if you go into a bar where nearly everyone is smoking, someone who doesn't want to be around the smoke should move. But are you saying that if a group of people are sitting around a bar counter and one person decided to smoke, the others should have to either deal with it or move? If the smoker needs a cigarette so badly that he is willing to bother everyone else with it, why can't he just smoke somewhere else?
Well, quite frankly, being bothered by something as inconsequential as second-hand smoking at bars, and whining about it, is something I consider to be very unmanly. To be quite honest, the more time I kill around this forum, the more I start to wonder whether there actually are some cultural differences, where I previously thought there to be none.

Care to elaborate? The only reason that I can see is that now all the smokers have to go outside, they drop their fucking butts everywhere. It's bizarre. People who usually wouldn't litter, constantly throwing non-biodegradable rubbish on the ground like it isn't actually litter at all.
Guess what? I like going to bars to meet up with friends and have a beer. I also like to go and see bands play. And I fucking hate coming out of these places stinking like fucking cigarettes. And the law has been passed here banning that shit, and it'll (one day) be passed in the USA as well, so suck it.
Smoking bans have been passed in a few cities around the U.S. already. New York has had it for a while now, and Philadelphia more recently. Other than those, I don't know any specific cities that have banned smoking indoors, though.
