Sadistik Exekution - crazy bunch of cunts they are.
Also this Canberran (kind of hardcore, but they're alright) band called 4 Dead. They singer looks like a fucking emo poof but holy shit he goes apeshit live. He once dropkicked a girl in the stomach, ran back on stage, punched the guitarist in the face then jumped into/cut himself up into/on the drums

I'd definitely say Gorgoroth although Mayhem's close. Whoever can crucify women onstage and torture a man for several hours is not exactly sane.
