what does this term actually mean? Is it derogotary? Does it mean a mediocre, cliche death metal band that things they're good because they can roar a lot? That's what I thought it meant, but is it a general term of mockery?

I think it's mockery for people picking, laughing at the genre death metal.

Generally refers to typical, derivative, gutteral vocals. The term would be used either in a neutral quick way to describe their sound or more than likely as a derogatory comment by those who aren't fans...

Have a bad childhood?

Yeah I usually only hear it in a derogatory term, either from retards or metal newbs who don't like growled vocals.

it means shitty vocals. some bands really do sound like cookie monster is screaming in them. amon amarth does, but i still like em ok

I don't think the term cookie monster vocals has to mean that the vocals are shitty... i've seen it used juist as a description of growly type vocals, not necessarily implying that the vocals are bad.
