I had this same problem for quite some time. Quite frankly it ended up being that I simply wasn't that comfortable with those first few girls I was with. Went 6 hours one time with the first girl as she was desperate to make it happen...I ended up doing my best to fake it after everything was numb. All of those relationships lasted at least a year as well...it just never happened with them.

Been absolutely fine with the last few girls I have been with, but let me tell you when it happens the first time after it never fully reaching that point for so long...it will catch you quite off guard.

Might be easier in certain positions. I tend to finish with the girl on top since I'm the most relaxed and can focus on achieving my own orgasm and no longer hers.

Yes. If you go long enough without finishing, it can get completely desensitized. It's not actually really numb, but you can't feel much and continued fucking won't really do much.

That's horrible man. I don't know what it is with women, but so many of them have such a tendency of trying to manipulate situations, and don't hesitate to directly start intricate plots using and lying to their 'friends' to achieve 'x' mundane thing or simply for scorn points.
