I think basically, what it comes down to is the fact that if the Special Instructions are not followed, the customer more than likely has the right to a refund, no?

It's definitely fun to see some of these requests though.

I don't think so. Otherwise what's to stop someone from writing in "kill yourselves and kill everyone you know?" Special instructions implies that the dude ordering needs to let the delivery guy know if he needs to go to the back door of the house or something. People take advantage of that box and for fun the bored delivery guys follow the "joke" instructions and technically they can't get in trouble for doing so.
